Medical appointments can be challenging and intimidating. Many people feel nervous, anxious, or unsure before an appointment, which is why it's best to prepare.
The first hurdle is knowing what questions to ask and when.
This tool helps you, or whomever is most involved in your care (i.e. informal caregiver), prepare for an upcoming appointment by prompting you to create a list of questions for you to ask your healthcare provider based on the reason for your visit. Appointments are often short, usually only 10 or 15 minutes, which can affect how many questions you ask and the answers you receive. Be aware that there may not be enough time to discuss everything.
